Who qualifies

Final eligibility is decided by Pennsylvania and your loved one’s health plan.
Your loved one
- Is 21 or older
- Has Pennsylvania Medicaid, or may qualify. Many people on Medicare qualify too.
- Needs help with things like bathing, dressing, meals, walking or medications
- Lives in Philadelphia, Delaware, Montgomery, Bucks or Chester County
You, the caregiver
- Are 18 or older and can work in the U.S.
- Can pass a background check
- Complete our free training
PA Health & Wellness does not pay a caregiver who is the person’s spouse, legal guardian, representative payee or power of attorney. Another relative or friend can step in, or we can provide a caregiver.
Already getting home care hours?
You choose who gives the care
If your loved one already has approved Community HealthChoices hours, they can move them to Philadelphia Care Core and have you become their paid caregiver. We help with the call.
Questions families ask
Straight answers
How much will I actually get paid?
You earn an hourly wage for each approved hour of care you give. How many hours depends on your loved one’s needs and what their health plan approves, so the estimator shows a range. Your coordinator confirms the rate and expected hours on your call.
Is this a real job?
Yes. You are hired by Philadelphia Care Core as a caregiver. You log your hours and we handle payroll, taxes and paperwork.
My loved one has Medicare, not Medicaid. Can we still do this?
Medicare alone doesn’t pay for this care, but many people with Medicare also qualify for Medicaid based on income and care needs. We help you check and apply.
Can a spouse or power of attorney be the paid caregiver?
No. PA Health & Wellness does not pay a spouse, legal guardian, representative payee or power of attorney to provide this care. Another relative or friend can, or we can provide a caregiver.
Does it cost my family anything?
No. When your loved one is eligible, approved home care is covered by Medicaid. We never charge families a fee.
Can I keep my other job?
Often, yes. Many caregivers fit care hours around other work. We go over scheduling with you.
Will a nurse come to the home?
Yes. A registered nurse visits to set up the care plan and checks in regularly to support you.
